Subject: commit - doxygen related cleanups and hacks

\page and \ref do what is needed to handle everything with one config file
and one global run of doxygen, replacing use of tag files and HTML links.
Found that HTML commands works way more reliably than the doxygen commands,
and those have the added advantage of being wellknown by everyone (I think).
Added the PieceTable document and enabled a few class descriptions.
