Added some configurability to the page numbering stuff. Now you can do
both headers and footers, and there's a seperate function in FV_View for
_insertHeaderFooter. You can also pass properties to header/footer in
general. However, there are still big bugs, which need to be resolved
before I add a real UI.

1) It doesn't display as a header/footer. It just displays all the
formatting as if it were a section at the end. Only by closing and
reopening the document can you see the correct behavior.

This is the biggest bug. It is a formatter problem, which has never been
dealt with because we never inserted footers on the fly before. I'm
willing to work on it, if someone points me in the right
direction. Please help.

2) It asserts on use, and crashes on undo. This may be just a small
oversight, or something big. Most likely, I've missed it b/c I'm new to
this part of the code. Again, suggestions welcome.
