Subject: commit -- xml and the preprocessor

Remove lots of duplicate preprocessor magic for XML by moving XML includes to a
dedicated file. If a file uses XML and does not include another file that does,
it should include "ut_xml.h", which will include the correct one of libxml2 or
expat headers.
