Implement Header/Footer Margins. Header/Footers also now correctly update
when you change the page-margins and page sizes. SegFault when dropping
back to 1 page then going to more than 1 page is fixed.

This really improves the header/footer infrastructure. BTW I define Header
margin as the distance from the top of the page to the start of the
header.

I define the Footer margin as the distance from the bottom of the text to
the start of the footer.

More fixes to follow...
