Lots of Footnote work. Footnotes expand,contract, can be clicked in,

Now you can't enter a footnote via cursor left/right arrows. You can click
in them. They expand and contract correctly when their height changes.

Still lots more work required but it should be straight forward until I
tackle undo of insert footnote.
