Improvements to document locks while printing. Improvements to cursor
wait. fix bug 608. Fix crash bug on print preview with a header/footer.
Now if you load fv_View.cpp into abiword and do "select all" you don't get
garbage all over the place. This fixes similar bugs in big documents.
