Commit: remove some table stuff from file format

    This will make all of your past several weeks worth of table-enabled
    documents invalid. To make them valid again, remove </endcell> and
    </endtable> from them.

    This makes the file format prettier look more like docbook, html, wml,
    xsl-fo, ... We don't need explicit endcell and endtable entities because
    we can insert the proper struxes into the piecetable on </cell> and
    </table>, respectively.
