More work to fix 4180. Contains the text correctly now but the toolbars
and rulers are still wrong.
    
I'm still finding bugs with tables spanning many pages. Particularly
expanding the tables to new pages and shrinking the size of the tables.

This work goes some way to fixing some of these issues.

Oh insert section break works again too.
