    Lots of Header?footer fix, including bugs 4991, 5001
                                                                                    
    The Header/Footer code was suffering bit rot through lack of attention.

    This commit fixes lots of bugs with headers/footers. Could our intrepid
    testers and crashers of abiword give headers/footers and good going over?

    Try lots of features and see what goes wrong. I suggest you may well find
    crashers and other funny things in headers/footers combined with section
    breaks.
