Fix 4649 (Crash on load on doc with table) and some other Table related
bugs

The doc from 4111 loads OK and doesn't oscillate. My Grant applications
load and work OK. How I've found I can get two tables overlapping in bug
4111 _ I don't know why right now and I have to try hard to reproduce.
