    Fix insert row for all cases. Allow table controls to be outside the
    left and right margins.

              
    Prior to this commit insert row in a table with vertically merged cells
    would do very strange things.

    This is fixed now.

    Also allow the left and right-most cell markers to be dragged outside
    the left and right margins. MS Word allows this and I found a lot of MS
    Word documents where this happenned.

    This commit allows abi to manipulate those docs but I really wish MS
    Word forbid this coz I don't think docs should have tables outside the
    left and right margins.
