ut_types.h: added UCS_LIGATURE_PLACEHOLDER (0xf854) to use instead of
the 0xfeff

ut_contextGlyph.h/cpp: removed stale code, added mechanism for
checking glyph availability, extracted smart quote processing into
getSmartQuote(), so that when someone gets to implementing smart
quotes its ready to be used

gr_Graphics.cpp, gr_Win32Graphics.cpp, gr_Win32CharWidths.cpp: made
changes reflecting the new ligature placeholder

fp_TextRun.h/cpp: removed stale code, extracted some processing in
_draw() into inlined memberfunctions to improve readability, added
handling of ligature placeholders.
