Fix TOC bug when a page break is in the HEADING - fix's charlie's vexi TOC.
Charlie's vexi documentation had a TOC in it where the content of the TOC
for the heading 1 style was on a different line from it's label.
This was caused by a pagebreak in the same block. This commit remove's
pagebreak/columnbreak and linebreak runs from TOC's and fixes this bug.
