    More TOC work. "Insert Table of Contents" works to zeroth order.

    So now if you have a document with Headings styles, and do "Insert Table
    of Contents" the headings are found and placed together in the Table of
    Contents.

    This does work if you insert the TOC at the start of the document and
    may not work if you place th TOC immediately before or after a Heading
    Style.

    The TOC doesn't update you add a new heading or change a current one.
    It also doesn't get filled on loading.

    Never-the-less it's a nice proof of concept. I'm now sure this will all
    work as we want. AbiWord-2.2 will have auto-updating TOC's :-)
