Lots of TOC fixes.

Lots of nice stuff in this.

1. If you save a doc with a TOC it reappears upon reloading.
2. If you add a new heading, or add a heading style to a piece of text
it immediately appears in the TOC.
3. If you delete a heading or remove a heading style from a piece of
text it is immediately removed from the TOC
4. The format of the text in the TOC is the that of the style of the
TOC.

Next a "......" TAB and page number for each entry in the TOC.
