TOC's have Heading numbers. More TOC bug fixes.

Numbered Heading styles now get their Numbers included in the TOC.

The TOC now reflects the Numbering of each Heading. This can be made more
flexible but doing a nice GUI for this will be hard. I'll go with this for
now.

Next I'll do a GUI for "Format TOC".
