Fix a few more TOC bugs including the crash outlined in 7106.
The crash is fixed now. As is the bug where deleting the paragraph
following a TOC does not put the content from the following paragraph into
the TOC.
Bug 7106 now exposes a refresh bug. I'll get on to this later
