Fix for 6809 crash inserting footnote/ednote fields from field dialog. Not
text\ed on windows and osx.
I fixed this by disallowing the insertion of these fields from the fields
dialog. They're spefically tield to footnote/endnotes.
Unfortunately this involved changing platform specfic code. It works fine
under linux. It should work for windows and OSX too but I haven't actually
tested these.
Windows and OSX builders should test and make sure it's OK.
