Last of the commits

The last of the commits I made last week. Abi almost compiles inside
the VS.NET 2003 IDE now, when using the new MSVC71 module.

Creating MSVC71 module to hold Visual Studio.NET 2003 solution files.
More MSVC71 build work. Remove MSVC6 PCH cruft. Copy MSVC-specific
headers automatically.

Simplify MSVC6 build process. No more manual copying of files should be
required.
