import revision markup for paragraphs

I added new append method to the PT and document appendLastStruxFmt()
which allows to change fmt of lastly appended strux of given type. We
need this in rtf import, for example, where some paragraph props can
be signaled by fmt markup to the \par keyword that concludes
paragraph (\par is really rtf representation of \r).

files: pt_PT_Append.cpp, pt_PieceTable.h, pd_Document.h/cpp,
ie_imp_RTF.cpp.
