I've made some changes re. RTF and images. I reverted some previous changes
that I made to Diego's patch.
I've also enabled pasting of images now via the RTF clipboard. The
piecetable really *does* care if you call some methods when it's not
loading. So now we just do a really simple switch on whether the input file
is NULL or not. If it's null, we're pasting from clipboard and should use
FV_View::cmdInsertGraphic. If it's not null, use the pd_Document->appendXXX
methods.
