applied 'Footnote/Endnote Reference' style to the anchor,
'Footnote/Endnote Text' style to the body, and changed the default
footnote type to numeric instead of numberic with brackets. Fixed a
typo in the definition of Footnote Text.
There is a bug in the FormatFootnote dialogue which only allows
'restart each section' or 'restart each page'. In fact this is a
three-state logic and the 'do not restart' variant is not handled by
the dialogues (checked for Unix and win32).
