added a number of new insert*BeforeFrag() methods to document/PT
classes; a new findFragOfType() to doc.

added a number of new methods to Word importer including
_appendFmt(), _appendStrux(), _appendSpan() and _appendObject(). All
future code must use these methods instead of calling getDoc()-
>append*().
