    Quick printing. Footnotes display in normal mode.

    Similar idea to quick zoom. Don't rebuild the layouts on printing. Just
    pass a print graphics clas through the pDA->pG pointer in the draw methods
    of the fp_* classes.

    There quite a few issues resolved for non-print view modes. But they're
    fixed now.

    Also finally tracked down the bug that prevented footnotes being displayed
    in normal mode.

    This should be backported to 2.0 once stress tested too.
