commit (HEAD): optimize strux/object insertion in revision mode

    This gets rid of the separate strux insertion and fmt change in
    revisions mode as Martin suggested (the same for objects). As far as
    span insertion is concerned this will require a bit more work because
    the insertSpan() methods do not at present allow to pass in
    attrs/props. Will fix this later.
