Update for 2.2 on Unix, edit a little in Unix instructions, update MSVC
instructions, update win32 instructions with respect to new nsis and
zlib handling (rp), merge divergences from various commits; the docs
should be in sync now

A note, I actually could've used revisions to merge documents today. I
didn't because I'd already merged everything by hand, but I could've, it
seemed to work. Good work Tomas!
