I'm hoping that there's a way to manage this so that we can just add
headers/footers on demand as needed, like Sam suggested.

If so, then legacy documents wouldn't need any special code. Ditto for
import/export of documents from other file formats which may not even have a
concept of headers & footers.

Yeah, that would look different, and not necessarily in a good way. I've
lost track of the reason why we always need the boxes. Do we?
