    Well, the Insert menu is the obvious place to put the Insert
    Header/Footer commands.

    The Delete commands would seem to logically go on the Format
    Header/Footer dialog, I think.

    What are the shortcut keys? In the absence of a menu command, they
    aren't easy to find.
